Many older revolvers, especially those in service use, may have been used more roughly that we might today. One of the things that can mess up a revolver is "snapping" the cylinder shut by flipping the wrist to close. Very common to see that in older cops-and-robbers movies... That will loosen the crane and can cause "spitting". Another is to pull the hammer back very quickly when shooting single action. With a heavy cylinder, like the Model 27, that can definitely beat up the locking cam eventually.
